What Are PCA Services?
Personal Care Assistance (PCA) Services provide assistance and support for people with disabilities of all ages to live in their own homes rather than in institutions or facilities. PCA services assist people with a physical, behavioral or mental health disability live independent lives.
A PCA/CFSS worker provides hands on assistance and/or reminders and prompts with:
Activities of Daily Living (ADLs): Dressing, Bathing, Grooming, Eating, Toileting, Transfers, Mobility and Positioning.
Instrumental activities of daily living (IADLs) related to living independently in the community: Go to medical appointments, participate in the community, complete household tasks necessary to support the person with an assessed need (e.g., planning and preparing meals or shopping for food, clothing and other essential items)
Observation and redirection of behaviors
Health-related procedures and tasks: Procedures and tasks performed by a PCA that can be delegated or assigned by a health care professional licensed under Minnesota state law.
How Is a Need for PCA Services Determined?
Contact your county of residence Public Health Nurse (PHN) and request to be assessed for PCA services. What you report and demonstrate at your assessment will help the PHN assessor make an accurate determination of your eligibility.
How Do I Get Started With A+ Home Care?
Once it is determined you are eligible for PCA services, you or your Responsible Party (RP) will be asked to designate an agency, like A+ Home Care, Inc to provide services. The PHN will then send us your PCA Assessment and Service Plan (SP). DHS will issue a Prior Authorization (PA) with a start and end date of your services.
You or your assessor will contact our agency to arrange for services to begin. Once the SP and PA are received at our office, one of our Qualified Professionals (QP)/RN’s will contact you to schedule an appointment with you to get the necessary paperwork signed by you and to write a care plan with your authorized cares. The date that our QP comes to your home is the date that your PCA can start to work with you.
If you are receiving PCA Traditional services, we will then give you referrals for you to contact. If you are on PCA Choice, the PCAs coming with you must complete the Minnesota Individual Personal Care Assistant (PCA) Standardized Training, pass the MN Background Check and be set up with payroll at our office.
Criminal background checks are required for all employees, and all employees are bonded, insured and supervised.
Only after our Qualified Professional (RN) completes your “Open Visit” can your PCA start working with you.

What Are PCA/CFSS Services?
CFSS: DHS is in the process of replacing PCA with CFSS. CFSS includes consultation services (PCA does not). All people who use CFSS must select a consultation services provider. Your assessor will authorize consultation services and give you information about how to find a list of consultation services providers. All consultation services providers must have a contract with the state to provide consultation services.
